If you are a Registered Nurse with Compact State licensure, and would enjoy a 30 hours per week (Friday - Sunday, 10 hours per day), this job could be for you. In this role, you would monitor Complex Chronic Disease Patients in a case management role via tele-monitoring. There is an opportunity to work from home following successful training. **A Day in the Life** You would conduct a telephonic assessment of a patient's reported symptoms, disease process, and biometric status, triage of critical situations, and educating the patient on their conditions, co-morbidities, medications and lifestyle. You'd be part of a team that monitors patients in the Patient Advocacy and Support Service Center which includes: Addressing all Biometric alerts daily. Prioritizes calls by acuity. Call patients to verify reported symptoms biometrics. Follow-up with providers and case managers as required per protocol. Outreach calls with patients to acquire past medical history with an introduction to our program. Scheduled outreach calls, after a hospitalization, to educate the patient on their disease process, medication reconciliation, ensure provider follow-up, and establish any barriers to care. Documenting nursing interventions taken during telephonic outreach/assessment. Updating patient records based on information received from providers/case managers. Reviewing and analyzing generated Trend Reports within department software prior to telephonic assessment. **Must Haves - Minimum Requirements** Bachelor's Degree; preferably in Nursing or relevant health related field. Registered Nurse. Compact State Licensure Bilingual (English/Spanish) preferred. Five or more years of experience in a variety of nursing fields, especially chronic health conditions. Cardiology, critical care, and emergency room experience beneficial. High level of competency in pathophysiology, pharmacology, disease specific assessment(s) and patient/family education. **Nice to Have** High level of autonomy and ambition in performing his/her daily duties with minimal supervision. The Patient Advocacy Nurse Specialist will have excellent time management and prioritizing skills. Preferred experience in Heart Failure, Diabetes Mellitus, and Chronic Obstructive Pulmonary Disease. Excellent triage skills assessing critical situations. Although Medtronic Remote Monitoring Program is not meant to be an emergency service, such situations may arise. Excellent critical thinking skills and problem solving skills. Excellent telephone etiquette and interpersonal skills. Excellent written and oral communication and documentation skills. Strong competency in computer skills, such as word processing, spreadsheets and e-mail (Microsoft Word, Excel, and Outlook). Basic typing skills (minimum 35 words per minute).
