You'd be part of a team that monitors patients in the Patient Advocacy and Support Service Center which includes: Addressing all Biometric alerts daily. Prioritizes calls by acuity. Call patients to verify reported symptoms biometrics. Follow-up with providers and case managers as required per protocol.
