Job Description Job description The Registered Nurse Case Manager (NCM) is responsible for actively coordinating and monitoring all clinical aspects of the Program intervention. The NCM ensures the appropriate selection of participants and monitors participant safety with active oversight of the Medical Director. The NCM assesses the participants clinical response to the interdisciplinary treatment plan and funnels communication to the appropriate specialist and/or Medical Director or referring physician. Provides leadership of weekly case conferences, and assures safe care transition of participants upon graduation. The NCM functions as a superuser of the electronic health record, DiaWeb, and participates as a member of the transdisciplinary team to ensure the quality and clinical effectiveness of the Program.
